2021- 2022 Boarding Students Enrollment Agreement
I hereby apply for enrollment of my student athlete (Enter Student Name Below) as a student-athlete at TLAP Sports Academy Preparatory for the school year from August 1, 2021 to March 30, 2022.
The terms and conditions of said enrollment are set forth with this 2021-2022 Enrollment Form and Agreement (“the Agreement”) as follows:

- Boarding Students (9th - 12th Grade & Post-Grad H.S.): $17,390
- Payment Plan: Paid in (8) installments throughout the academic year.
- Registration Deposit: $3,500 - (Due upon registration enrollment.)
STUDENT-ATHLETE MEALS, HOUSING & UPGRADES: Standard shared rooms and meals (breakfast, lunch and dinner) annual team uniform, practice & casual wear gear, team build activities & events, and travel tournaments transportations are all included in the tuition.
Tuition Total Cost: $17,390

Payment plans are for the convenience of parents, In the event of the early withdrawal of a student, parents are responsible for the entire balance of the year’s tuition. If tuition is received by TLAP Sports Academy Preparatory on the 1st of the month, there will be a $100 charge for late payment. I further understand and agree that there will be a $100 charge for each returned check. I understand and agree that any payment not received with 5 days of the due date may result in the voiding of the contract by TLAP Sports Academy Preparatory. I also understand student athletes will not be allowed to attend program classes/training/events when tuition is 7 days overdue. A student athlete’s tuition payment must be current for him or her to be permitted into program.
I further understand, acknowledge and agree that the TLAP Sports Academy Preparatory has a limited enrollment. By executing this agreement, TLAP Sports Academy Preparatory agrees to provide a place for my student athlete ton the 2021-2022 team, which may deprive another student athlete of the privilege of enrolling at TLAP Sports Academy Preparatory. Further, in the event that my student athlete withdraws from TLAP Sports Academy Preparatory, TLAP Sports Academy Preparatory may be unable to enroll another to fill my student athletes place in the team. Hence, I agree that all obligations shall be continued, and this agreement shall not be terminated for any reason other than and absence of withdrawal of my child due to a catastrophic injury or illness to the minor child or to the undersigned.
TLAP Sports Academy Preparatory reserves the right, at its sole discretion, to suspend or dismiss the above-named student athlete I the athlete’s presence at TLAP Sports Academy Preparatory would be detrimental to the student athlete, the team or TLAP Sports Academy Preparatory. In such event, parents are responsible for the remainder of the academy years tuition and fees.
